Declaration of COVID-19-Related Financial Distress – This form should be used by tenants who are unable to pay rent due to financial distress arising from and/or related to COVID-19. If you receive this declaration form from your landlord, you must sign and return it to the landlord within 15 business days of when you received it in order to avoid eviction for non-payment of rent due to COVID-19-related financial distress.
